Key Responsibilities
- Manage a designated caseload of post-release services while assisting with home study reports to evaluate risk, safety, and appropriate child placements.
- Ensure adherence to established policies, procedures, and mandated timelines.
- Collaborate closely with care providers to verify information for successful sponsorships.
- Assess the capability of prospective sponsors to create a safe and nurturing environment.
- Identify additional resources or support for children and their sponsors as needed.
- Conduct both virtual and in-person home visits to verify child safety and well-being.
- Employ trauma-informed approaches and brief therapeutic techniques in interactions with children and families.
- Provide continuous case management through assessments, planning, and psychoeducation.
- Maintain precise documentation and uphold confidentiality in accordance with program regulations.
- Report any significant incidents or concerns to the relevant authorities.
- Engage in training, team meetings, and quality improvement initiatives.
- Travel as necessary for in-person home studies and post-release visits, which may include occasional evening and weekend work.
- Strong writing abilities to create comprehensive and professional assessment reports.
- Excellent verbal and written communication skills to engage effectively with families, caseworkers, and stakeholders.
- Proficient in Microsoft Office and adept with data entry systems.
- Capacity to manage confidential information with the utmost discretion.
- Fluency in both English and Spanish is mandatory.
- Bachelor's degree in Social Work (BSW) or a related field, such as education, psychology, sociology, or another behavioral or social service discipline.
- A minimum of one year of experience in case management within a social services environment.
- Familiarity with trauma-informed interventions for youth, especially within immigrant populations.
- Knowledge of family preservation practices, kinship care, child welfare policies, and applicable state and federal regulations.
