Key Responsibilities
- Conduct annual performance evaluations and supervise assessments of clinical personnel.
- Manage staff scheduling, coordinate vacations, and ensure adequate coverage.
- Facilitate the orientation process for new field staff in partnership with Human Resources.
- Collaborate with leadership to ensure alignment of home care and hospice services with financial and operational objectives.
- Oversee patient transitions between home care and hospice services, ensuring seamless continuation of care.
- Provide comprehensive supervision of all patient care services and staff, including:
- Collaborative patient assignments
- Continuous assessment of patient needs
- Management and updates of care plans
- Coordinate the daily activities of field staff and ensure efficient task management.
- Support quality management initiatives in conjunction with the Quality Director.
- Conduct utilization reviews and guarantee adherence to professional practice standards.
- Address patient concerns proactively, seeking efficient resolutions.
- Perform skilled nursing visits and participate in on-call rotations as necessary.
- Lead interdisciplinary meetings and conduct training sessions for both new and existing staff members.
- Monitor clinical schedules, documentation, and productivity metrics.
- Assist in hiring processes, including creating job postings, conducting interviews, and onboarding new staff.
- Ensure compliance with Medicare Conditions of Participation and all regulatory requirements.
- Offer ongoing coaching and leadership to staff, fostering a professional and positive work atmosphere.
- Associate's degree in nursing is required; a bachelor's degree is preferred.
- Active RN license in good standing.
- Previous management experience is preferred.
- Strong knowledge of home health and hospice regulations.
- Excellent communication, leadership, and problem-solving abilities.
- Proficient in electronic medical records (EMR) systems.
- Flexibility to work weekends as needed.
- Reliable transportation along with a valid driver's license and proof of insurance.
