Job Overview

Key Responsibilities

• Perform screenings and assessments to evaluate eligibility for case management services.
• Create and execute individualized care plans tailored to the specific needs and goals of each beneficiary.
• Continuously monitor and refine care plans to ensure their ongoing effectiveness.
• Advocate for beneficiaries by facilitating access to crucial healthcare services and resources.
• Provide insights on available community programs and support networks.
• Review and process authorization and referral requests while ensuring compliance with established policies.
• Maintain precise documentation within the medical management system.
• Collaborate with healthcare professionals and internal teams to ensure cohesive care plan execution.
• Provide mentorship to non-clinical staff to support operational goals.
• Uphold professionalism, dependability, and effective communication in all professional interactions.
  
  

Required Skills

• In-depth knowledge of case management principles, HIPAA regulations, URAC standards, and medical coding.
• Strong problem-solving skills with the ability to navigate complex situations and devise effective care strategies.

• Proven organizational skills with the ability to manage multiple priorities in a dynamic environment while attending to detail.

Qualifications

• Current, unrestricted state license as a Licensed Clinical Social Worker (LCSW).
• At least two years of direct clinical care experience.
• Ability to obtain a URAC-recognized case management certification within four years of employment.
• U.S. citizenship and capability to pass a Department of Defense (DOD) background investigation.
