Key Responsibilities
- Perform screenings and assessments to evaluate eligibility for case management services.
- Create and execute individualized care plans tailored to the specific needs and goals of each beneficiary.
- Continuously monitor and refine care plans to ensure their ongoing effectiveness.
- Advocate for beneficiaries by facilitating access to crucial healthcare services and resources.
- Provide insights on available community programs and support networks.
- Review and process authorization and referral requests while ensuring compliance with established policies.
- Maintain precise documentation within the medical management system.
- Collaborate with healthcare professionals and internal teams to ensure cohesive care plan execution.
- Provide mentorship to non-clinical staff to support operational goals.

- In-depth knowledge of case management principles, HIPAA regulations, URAC standards, and medical coding.

- Current, unrestricted state license as a Licensed Clinical Social Worker (LCSW).
- At least two years of direct clinical care experience.
- Ability to obtain a URAC-recognized case management certification within four years of employment.
- U.S. citizenship and capability to pass a Department of Defense (DOD) background investigation.
