Key Responsibilities
As a Licensed Practical Nurse, your core responsibilities will include:
- Collecting and documenting patient information to support healthcare providers in assessments and care planning.
- Administering prescribed medications and treatments as directed by medical professionals.
- Monitoring and documenting vital signs, medical histories, and patient symptoms.
- Assisting with physical examinations and various medical procedures as necessary.
- Educating patients and their families on relevant healthcare topics to encourage overall wellness.
- Maintaining precise and timely documentation pertaining to patient care.
- Collaborating with school nurses, WIC colleagues, DCF, and other medical departments as required.
- Completing necessary paperwork for external organizations (e.g., school physical examination forms).
- Triage patient calls using established protocols to ascertain appropriate care, including home management or medical visits.
- Providing patient and family education, which includes information on after-hours care options and referral services.
- Contacting specialists to secure necessary documentation and facilitate home therapies as recommended by a provider.

- Proficiency in using electronic medical records; familiarity with eClinicalWorks is preferred.
- High School Diploma or GED.
- Current LPN license in the state of Connecticut.
- Previous experience in pediatrics is preferred.
