Remote LPN - Licensed Practical Nurse - LVN Care Coordinator

Key Responsibilities

• Oversee a patient census with a focus on resolving gaps in both clinical and non-clinical care.
• Identify and coordinate community resources that can aid patients in managing their health.
• Educate patients to enhance their health literacy and assist in the management of chronic conditions.
• Manage medication, including addressing potential concerns, reconciling medications, providing adherence support, and coordinating refills.
• Ensure timely delivery of essential services such as Home Health, Durable Medical Equipment (DME), Home Infusion, and other critical patient needs.
• Engage patients in meaningful dialogue to comprehend their needs, respond to inquiries, and offer compassionate support.
• Facilitate referrals and appointment scheduling to ensure seamless patient care coordination.
  
  

Required Skills

• Active Multi-State/Compact License (LPN) (NLC) (LVN)
• Proficient in Microsoft Office Suite
• Exceptional communication skills, both written and verbal
  
  

Qualifications

• Proven experience in patient care coordination or a related field.
• Familiarity with Epic EMR is a plus.
• Bilingual skills and additional single-state LPN licenses are advantageous.
