Key Responsibilities
- Oversee a patient census with a focus on resolving gaps in both clinical and non-clinical care.
- Identify and coordinate community resources that can aid patients in managing their health.
- Educate patients to enhance their health literacy and assist in the management of chronic conditions.
- Manage medication, including addressing potential concerns, reconciling medications, providing adherence support, and coordinating refills.
- Ensure timely delivery of essential services such as Home Health, Durable Medical Equipment (DME), Home Infusion, and other critical patient needs.
- Engage patients in meaningful dialogue to comprehend their needs, respond to inquiries, and offer compassionate support.
- Facilitate referrals and appointment scheduling to ensure seamless patient care coordination.
- Active Multi-State/Compact License (LPN) (NLC) (LVN)
- Proficient in Microsoft Office Suite
- Exceptional communication skills, both written and verbal
- Proven experience in patient care coordination or a related field.
- Familiarity with Epic EMR is a plus.
- Bilingual skills and additional single-state LPN licenses are advantageous.
