Key Responsibilities
- Manage a designated patient census, employing a resolution-focused strategy to address clinical and non-clinical care gaps.
- Identify and connect patients with essential community resources that can elevate their care and overall well-being.
- Deliver effective patient education regarding chronic condition management, fostering health literacy and self-management capabilities.
- Conduct comprehensive medication management, identifying potential issues, reconciling medications, ensuring adherence, and overseeing refills.
- Facilitate timely access to vital services for patients, including home health, durable medical equipment (DME), and home infusion.
- Engage in open communication with patients, addressing their inquiries and providing compassionate support.
- Assist with referrals and appointment scheduling to ensure seamless coordination of care.
- Active Multi-State/Compact License (LPN) or (LVN).
- Proficient in Microsoft Office Suite and strong technical aptitude.
- Exceptional written and verbal communication abilities.
- Capacity to work autonomously in a remote setting.
- Ability to maintain prolonged periods of sitting and effectively utilize a telephone.
- Capability to accurately process and document detailed information during phone discussions.
- Bonus Points: Experience with the Epic electronic health record (EHR) system, bilingual proficiency, or additional single-state LPN/LVN licenses.
