Key Responsibilities
- Conduct comprehensive psychosocial evaluations to assess clients’ mental health needs.
- Create and execute tailored treatment plans in partnership with healthcare teams.
- Establish and nurture strong therapeutic connections with patients, guardians, and families to facilitate recovery resources.
- Deliver individual, family, and group therapy, utilizing various therapeutic methodologies.
- Work closely with the treatment team to address discharge planning and other critical matters.
- Advocate effectively for clients to ensure they access the necessary resources and support.
- Collaborate with healthcare colleagues to provide integrated and comprehensive care.
- Offer crisis intervention services during psychiatric emergencies.
- Educate clients and their families about mental health conditions, treatment alternatives, and available resources.
- Maintain precise and confidential client records.
- Facilitate access to community resources and supportive services.
- Monitor and assess client progress, modifying treatment plans as required.
- Oversee or assist with patient transportation services as needed.
- Initiate or supervise guardianship and involuntary commitment proceedings when applicable, following ethical standards.
- Support fellow staff members in ensuring respectful and clinically appropriate patient interactions.
- Provide clinical insights and recommendations during diagnostic and treatment review conferences.
- Assist in addressing social service needs as they arise.
- Minimum one year of clinical experience in direct patient care as a Licensed Clinical Social Worker.
- Versatile experience across diverse settings such as detention and correctional healthcare, residential healthcare, hospitals, outpatient clinics, nursing homes, or similar supervised medical environments.
- Experience working with both pediatric and adult populations.
- Strong communication skills in English, both verbal and written.
- U.S. Citizenship or Permanent Resident status.
- Reliable transportation to Concord, NH, or willingness to relocate.
- Flexibility to travel at short notice or serve in various medical units.
- A Master’s degree in Social Work (MSW) from an accredited institution.
- Current licensure in New Hampshire with no disciplinary actions on record.
- Basic Life Support (BLS) Certification from a recognized organization (AHA, Red Cross, ASHI).
