Key Responsibilities
- Oversee and coordinate clinical services, providing and directing nursing care to patients in their residences.
- Deliver clinical services within your scope of practice, adhering to state regulations and the patient’s individualized care plan, in collaboration with the healthcare team.
- Manage referrals, ensure appropriate clinician assignments, evaluate patient orders, and schedule start-of-care visits.
- Coordinate patient home health benefits, determine medical necessity, and manage insurance approvals.
- Continuously assess patient needs to ensure care is personalized and meets eligibility criteria, following physician orders.
- Review assessments and plans of care, offering consultations and recommendations to clinicians.
- Strong clinical management capabilities.
- Ability to provide individualized patient care.
- Excellent communication and interpersonal skills.
- Proficiency in managing referrals and coordinating healthcare services.
- Familiarity with state regulations and healthcare practices.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Valid driver’s license, vehicle insurance, and access to reliable transportation.
