Key Responsibilities
- Supervise and coordinate clinical services while providing and directing nursing care to patients in their homes.
- Evaluate patient orders, receive referrals, assign clinicians, and schedule initial care visits.
- Ensure coordination of patient home health benefits and manage insurance approvals, determining medical necessity.
- Continuously assess patient needs, ensuring care is customized, appropriate, and aligns with physician orders.
- Review daily assessments and plans of care, and provide expert consultations to clinicians.
- Current RN licensure in the state of practice.
- Current CPR certification.
