What are the responsibilities and job description for the Care Coordinator RN-Case Management-HHK-Sacaton position at Gila River Health Care?
Position Summary: Responsible for performing care coordination of assigned patients to ensure all care services are identified, scheduled, and delivered in a timely and appropriate manner. Performs care transitions based on appointments, testing, and referrals and coordinates with other facilities to obtain patient’s medical information prior to scheduled appointments. Communicates frequently with patients, family members, and other departments to provide patient care that is appropriate to patient’s condition and needs. Performs other customer service duties, including responding to patient complaints and providing instructions about procedures.
Critical Tasks:
- Participates in the development of individualized care plans for patients by performing assessments of needs based on health status, willingness to participate in care, family participation, and strengths and weaknesses.
- Coordinates with internal and external facilities to prepare patient information and test results prior to patient’s scheduled appointment.
- Verifies appointments and contacts patient or family members to follow up with referrals. Utilizes resources to locate patients and schedule appointments when patients do not respond to contact attempts.
- Maintains accurate documentations, such as charting, progress notes, and case records, of services and medical treatments in compliance with established policies and procedures.
- Collaborates with Registered Nurses and Case Managers to support dynamic needs of clinics and to coordinate seamless transition between patient’s home and hospital.
- Responds to all patient complaints by performing research for corrective action, escalating issues to appropriate parties, and identifying possible solutions.
- Reviews medical information and charts accurately to determine appropriate referral services based on patient needs assessed by the provider.
- Coordinates with Purchased Referred Care department to ensure timely referrals of patient’s specialty testing and consultation to other external healthcare facilities.
- Monitors and analyzes department performance data to evaluate the effectiveness of care coordination and management. Conducts researches to determine best practices and implement appropriate interventions to improve service quality.
- Provides education for patients to guide them through medical treatments.
- May supervise patient care activities of assigned team members by delegating duties and providing timely feedback.
- Performs other job-related activities as requested.
Required Qualifications:
- Bachelor’s Degree in Nursing preferred.
- 1 year of previous patient care experience in a primary care, acute care, home health, long-term care, or related healthcare setting required.
- Care coordination experience preferred.
- Current and valid State of Arizona Registered Nurse license.
- Current cardiopulmonary resuscitation (CPR) certification or ability to obtain the certificate within 30 days of hire. Exceptional communication, interpersonal, and customer service skills.
