Job Summary:
The Case Manager position supports the Director of Patient Care Services (DPCS) within the scope of all department functions.
Qualifications:
- Experienced in a medical office setting or equivalent.
- Knowledge and ability to follow appropriate skilled documentation under Medicare guidelines.
- Communicates effectively with Clinical and office staff involved in any given case in a constructive, goal directed, and professional manner.
- Ability to call doctors, patients, and pharmacies as needed.
- Must possess ability to understand departmental operations and procedures.
- Knowledge of insurance reimbursement procedure.
- Must have good phone skills, type 40 w.p.m., and have strong knowledge of Word, Excel, and computerized systems in general.
Responsibilities:
- Read clinicians notes randomly or as directed by the Director of Nursing.
- Makes necessary calls to notify clinicians of needed corrections to make.
- Ensures that patient plan of care is being followed by all clinicians.

- Ability to audit patient’s charts per episode making sure to prompt both supervisor and clinicians for discrepancies that may need an order.
- Ability to process data information and generate Plan of Care (485) with accuracy of all medications.
- Ability to learn and process Discharge OASIS as needed.
- Answer calls and get messages as needed.
- Performs such other duties as assigned.

Pay: $30.00 - $40.00 per hour
