Job Description
The Clinical Care Leader (CCL) plays a crucial role in delivering individualized nursing care in the home setting. They utilize the nursing process to assess, diagnose, plan, implement, and evaluate care, collaborating with inter-professional colleagues to ensure comprehensive care.
The CCL is responsible for documenting resident history, symptoms, medications, and care provided, adhering to applicable standards and regulations. They leverage technology to enhance resident care, communicate effectively with team members, delegate tasks, and assume overall nursing accountability for their assigned residents.
To excel in this role, candidates must possess effective communication skills, strong clinical reasoning, and critical thinking abilities to analyze data and respond to changing situations. A bachelor's degree in nursing is preferred, along with experience in long-term care or geriatric nursing.
Requirements
- Unencumbered RN license with the State Board of Nursing or multistate licensure in an NLC state
- Minimum of two years' long-term care or geriatric nursing experience
- Experience with Resident Assessment Instrument (RAI) process preferred
