- Assesses residents' physical and psychosocial needs, collaborating with healthcare professionals to develop effective care plans.
- Maintains accurate records of resident interactions, outcomes, and medication administration.
- Communicates effectively with residents, families, and healthcare teams to ensure comprehensive care.
Requirements:
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
- Associate degree in practical nursing is preferred.
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
