Key Responsibilities:
- Develop and implement individualized treatment plans that address the specific needs and goals of each client.
- Deliver educational resources and ongoing support to patients and their caregivers, fostering better understanding and management of health conditions
- Act as a key liaison between patients, caregivers, and healthcare professionals, ensuring clear communication and coordinated care
- Understand each patient's unique needs and preferences to provide a seamless and tailored experience, promoting adherence to treatment plans
- Keep thorough and up-to-date records, including progress notes, treatment plans, and assessments, to ensure compliance with clinical standards and regulations
- Offer confidential, non-medical counseling to patients experiencing chronic pain, helping them process their experiences and improve their well-being
- Master'sdegree in Social Work from an accredited institution
- Got a valid and active CA state license to practice as a Licensed Clinical Social Worker (LCSW)
- Minimum of 2 years of clinical experience to the table, with a focus on providing psychotherapy services
