Grace Pointe, a premier Senior Care Community located at 1919 68th Avenue in Greeley, CO, is currently accepting applications for a FT Activity Assistant in our Rehab Unit. The ideal candidate will have experience in working with seniors promoting an engaged active lifestyle. Salary range is $15.00-17.00 per hour and is based on years of experience as an Activity Assistant. Position is Monday thru Friday 8:00 AM-4:30 PM, with occasional weekends. Essential Duties & Responsibilities
- Assists and works with individual volunteers and volunteer programs at the request of the Activity Director.
- Conducts activity programs as planned to meet the functional levels, needs and interests of each patient as assigned by manager.
- Completes appropriate records indicating patient's attendance and participation in individual and group activities in an accurate and timely manner.
- Assists with escorting patients to and from activities.
- Assists in the development of the Activity calendars.
- Completes required forms and documents in accordance with company policy and state and/or federal regulations.
- Assist with family communication programs and visitations.
- Performs other duties as assigned.
