What are the responsibilities and job description for the Assessment Referral Coordinator position at Grandview Medical Center?
Position is viable in growing the referral base for admission to Inpatient Rehab. Works with in house physicians, case managers, rehab and others to educate and grow appropriate referrals. Works with surrounding entities such as physician’s offices and hospitals to grow referrals as well. Referrals are then evaluated based on stringent admission guidelines to ensure they meet medical necessity guidelines. Develops and works with 3rd party payors to facilitate pre-certification and payor authorization.
Education: Associate degree in nursing or licensed clinician from an accredited school of nursing. |
Experience: Two years prior clinical or medical experience is required. One year of rehab experience. Case Management experience preferred. Certification in rehab nursing preferred. |
Licenses/Certificates: Currently licensed by State of Alabama. Basic Life Support (BLS) |
