JOB SUMMARY:
This position provides direct support to an engaging young man with a number of specialized care needs who is served in our residential (24/7/365) program in Lamoille County. Exceptional training, mentoring, and ongoing support is provided to all members of this highly collaborative team.
Position may require evening, awake overnight, and/or weekend work. Shift differentials apply for work performed at those times.
ESSENTIAL FUNCTIONS:
- Promote the individual’s autonomy, dignity, and choices across environments.
- Provide personal care including meal preparation and assistance with activities of daily living.
- Assist the person to participate in his community and make authentic connections with community members.
- Support the individual to communicate effectively through careful observation, attentive ‘listening,’ and by using his preferred communication tool and approaches.
- Monitor and support the individual’s health by preparing meals according to his meal plan, observing for signs of illness, administering medications, and providing diabetes care, including insulin administration.
- Support the individual to effectively manage emotions and cope with stress.
- Nurture the person to develop and enjoy high-quality personal relationships.
- Complete work-related documentation in a timely manner, as instructed.
- Promptly communicate progress, concerns, and questions to the appropriate team members in support of excellent support provision and good team function.
- Attend team meetings and trainings as required.
Other Functions:
Other duties as assigned by the Program Manager or Program Director.
KNOWLEDGE AND EXPERIENCE: (Minimum education, experience, technical and communication skill levels required to perform the essential functions of this position.)
- High School diploma or equivalent
- Must have a valid Driver’s License, acceptable driving record, and be able to drive a van.
- Must be able to provide physical support in the shallow end of a pool.
- Must be able to walk for up to 45 minutes across ordinary terrain, including outdoors in all seasons.
- Must have basic computers skills sufficient to complete work-related documentation, communicate with coworkers, and submit timesheets.
- Must be able to provide diabetes care, including insulin administration. (Training is provided by an Agency nurse.)
- Must be able to listen well and use alternative communication tools. (Training is provided.)
- Must be able to maintain and project calm.
- Must be able to provide physical and personal care support.
- Must be able to maintain confidentiality.
- Must be able to remain flexible in response to the individual’s changing needs.
- Experience with individuals with intellectual and developmental disabilities is preferred but not required.

Work is normally performed in a climate-controlled home environment and in indoor and outdoor community settings such as stores, pools, and walking trails. Work may involve exposure to aggressive behavior and bodily fluids/waste. There is a risk of needle stick injuries.
PHYSICAL DEMANDS: (The physical effort generally associated with this position.)
Work may include occasional pushing, pulling, or carrying objects weighing approximately 50 pounds. Work may include physical assistance to the individual to maintain balance or get out of the water. Work sometimes involves driving.
