Job Description:
QUALIFICATIONS: BA (preferably in Social Work, Mental Health, Human Services or Psychology); Valid NYS Driver's License.
REPORTS TO: Building Principal > Transition Coordinator
JOB GOAL: Transition Specialists ensure students with disabilities successfully transition from school to post-secondary life.
JOB RESPONSIBILITIES: Responsibilities include attending CSE meetings, monitoring student progress, facilitating transition groups, and supporting students in vocational and adaptive skill development in school and in the community. You will also maintain relationships with community agencies, provide resources, and ensure students have the necessary tools for a successful transition, including digital portfolios and career plans.
LOCATION: Greenburgh Academy (108 Shonnard Pl. Yonkers, NY)
WORK SCHEDULE: 10-month position; 6.75 hours daily (full-time)

- No Weekends
- Day Shift
- Follows Student Schedule

SALARY: $34,649 to $39,869 (based on experience)
