Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY. The Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our Independent Living community (Fountaingate Gardens) an our Certified and Licensed Home Care Service Agencies.
Our innovative facility and growing services offer opportunities to advance your career. We are currently seeking a full time Social Worker, who will embody the passion and commitment required to deliver the ultimate care for our residents.
This position is Tuesday through Saturday from 8:30am-5:00pm.
RESPONSIBILITIES INCLUDE, BUT ARE NOT LIMITED TO:
- Candidate would perform social work duties which include but are not limited to discharge planning, helping the resident and/or his family with social, emotional, economic and rehabilitation concerns.
- Work with the interdisciplinary team to assist in planning care and providing interventions through direct services; and performing all other related duties as assigned.
Meets with new admissions and their family promptly; provides orientation to unit, explains policies and procedures of the facility.
Capable of providing direct social work services and counseling to residents and families, individually or in a group setting as appropriate, according to current standards.
Capable of completing all documentation according to the Policies and Procedures of the Department of Social Services.
Attends weekly Interdisciplinary Comprehensive Care Plan meetings and participates in completion of the comprehensive care plan.
Capable of completing MDS , SCREEN, and other evaluations as required by regulation.
Submits periodic reports to Director of Social Services related to individual residents, as well as special projects.
Assists family with application for Medicaid, SSI, Veterans or other benefits. Works with Finance Department to ensure continuation of benefits.
REQUIREMENTS INCLUDE:
MSW from an accredited School of Social Work.
Eligible to be registered as a Licensed Masters Social Worker (LMSW) with the New York State Education Department.
Minimum one year post graduate in a hospital or long term care setting preferred.
Experience in casework, group work and family therapy.
