Hamden Rehabilitation and Healthcare Center located in Hamden, CT. Is looking for an LPN/RN Clinical Liaison to join our team. We are a 153-Bed Skilled Nursing Facility, with short term care, long term care and a memory care unit.
Job Overview
The primary purpose of this position is to act as the facility liaison to the area's doctor's offices and senior centers, perform onsite evaluations of all potential patients referred to the subacute/short term rehabilitation units within a specific region. To collect medical, financial and psychosocial data necessary to determine viability of the patient for admission. Communicate clinical patient information to the Director of Admissions, where the final determination regarding the patient's acceptance for admission will be made. To network with discharge planners, case managers, physicians, for the purpose of positively and honestly promoting the facilities programs and increasing appropriate referrals from these sources. To assist in all functions of the Admissions Office including clerical and marketing tasks in accordance with current federal, state, and federal guidelines.
Responsibilities:
As the Clinical Admission Liaison/Evaluator, you are responsible and accountable to carry out your assigned duties and report directly to the administrator.
- Works within and outside the facility
- Perform onsite evaluations as requested by the facility in a timely manner in order to coordinate admissions.
- Act as a liaison between facility and medical providers such as senior centers, homecare agencies, etc.
- Create relationships with professional influencers and the local community and build confidence and preference for them to refer their contacts and clients to facility.
- Visit and make presentations to the influencers and their place of buisness, e.g,- doctor offices, hostipal discharge planners, case managers, social workers, home health agencies, assisted living community, rehab and adult daycare centers, etc.
- Develop formal weekly and monthly marketing planes, in conjunction with the administrator to identify and prioritize categories and specific targets of professional influencers and community organizations.
- Plan and coordinate events that will bring prospects and professional influencers to facility.
Skills
Must function independently, have flexibility, personal integrity and the ability to work effectively with the patients, their families, personnel and support agencies.
Must be a healthcare professional with acute or rehabilitation care experience. Must be an LPN or RN with a valid License.
