What You'll Do :

  • Provide comprehensive case management, care coordination, and advocacy for clients.
  • Establish and adapt Service Plans to address multiple needs and ensure progress.
  • Coordinate primary care needs within the continuum of care.
  • Assess and coordinate holistic care for all clients.
  • Document service coordination activities.
  • Maintain ongoing contact with clients and support systems.
  • Provide transportation support to minimize barriers.
  • Establish and maintain relations with civic groups, agencies, and professionals.
  • Arrange advocacy services for housing, legal matters, or relevant services.
  • Apply confidentiality regulations appropriately.
  • Provide timely reports and paperwork as required.
  • Attend all required meetings and trainings.
  • Maintain certifications and required training hours.
  • Attend community outreach events as needed.
  • Attend core trainings for case management and addiction.
  • Maintain identified certifications and complete annual training hours.
  • Perform miscellaneous job-related duties as assigned.
  • Requirements :

  • Healthcare Knowledge : A fundamental understanding of healthcare services and basic medical terminology.
  • Tech-Savvy : Proficiency in basic data entry computer skills.
  • Strong Interpersonal Skills : Ability to communicate courteously and effectively with individuals from diverse backgrounds.
  • Team Player : An industrious and cooperative nature to foster harmonious working relationships with clinic management, physicians, clinicians, and the public.
  • High School Diploma : Bachelor’s degree in a Human Services or related field
  • Preferred Qualifications :

  • Prior CPR / First Aid certification
  • One (1) year of working experience in case manager or medical field setting.
  • Knowledge of The Joint Commission regulations
  • Bi-lingual in Spanish, Nepali, French, Arabic
