Program Description
The Coordinated Specialty Care (CSC) for Psychosis Program provides evidence-based assessment, psychotherapy, medication management, employment and educational support, peer support, and family support services to children and adults who have recently began experiencing symptoms of psychosis as part of a Coordinated Specialty Care (CSC) team. The CSC Program uses a person-centered, team-based, and recovery-oriented approach to help individuals manage their new experiences while achieving their personal goals.
Job Description
Summary/Objective
The Family Peer Specialist will serve as a peer to the family members/natural support of the individual primarily receiving CSC services. The Family Peer Specialist will use their lived experience as a parent/caregiver of someone who has experienced psychosis to advocate for and represent our participants families while working as part of a Coordinated Specialty Care (CSC). The Family Peer Specialist will provide individual and group-based family peer support services to participants family members/natural supports, educators, and community members. The Family Peer Specialist will be thoroughly trained in family supportive services. This position is community/office/remote based.

- Provides a family peer perspective based on lived experience to the provider team, participants, and their caregivers/natural supports.
- Attends and participates in weekly team meetings
- Attends and participates in weekly management meetings
- Attends monthly consultation meetings
- Attends and participates in all assigned training for professional, program, and agency development.
- Assists with community outreach and education efforts
- Maintains a caseload of 40 families and meets expectations of 66% direct service hours monthly.
- Meets with families on a monthly, biweekly, or weekly basis depending on family needs.
- Provides education and support to participants families/natural supports as part of a clinical team.
- Co-leads Multi-family Groups in the evenings 5:30-7:30pm on a bi-weekly basis.
- Collaborates and coordinates on a weekly basis with other team staff including but not limited to psychotropic medication provider, mental health therapist, employment and educational specialist, and peer support specialist.
- Maintain timely weekly progress notes up to insurance standards.
- Submit required clinical documentation within 72 hours of service provided.
- Comply with all federal regulations, Health Insurance Portability and Accountability Act (HIPAA), for the privacy and security of client information.
- Maintain quality assurance standards, adherence to MediCal/MediCare documentation requirements and data collection for measured outcomes.
- Additional duties as needed.
Minimum Requirements:
- High School Diploma.
- Been a parent or primary caregiver of an individual with mental health challenges (preferably psychosis).
- Working knowledge of peer support roles and the history of service user movements.
- Experience supporting people of varying cultural & ethnic backgrounds in navigating the mental health system.
- Familiarity and knowledge of Sonoma County
- Willingness to work outside of office setting, in the community and homes of participants.
- Able to attend Team Meetings Tuesdays 10am-12pm.
- Able to rotate in co-facilitating MFG Groups on a bi-weekly basis Wednesdays 5:00pm-7:30pm.
- Open to working some afternoon/evenings 2pm – 6pm.
- Spanish speaking.
- Drivers License.
Preferred Qualifications:
- Completion of the Medi-Cal Peer Support Specialist Certification (or willingness to obtain).
Competencies:
- Uses personal experience as a caregiver of someone with psychosis to provide empathetic, peer-based support to families.
- Communicates effectively in Spanish and English while supporting families from diverse cultural backgrounds.
- Works closely with a multidisciplinary team to coordinate care and share family perspectives during meetings and planning.
- Manages a caseload, provides regular peer support, and maintains accurate clinical documentation in compliance with standards.
- Delivers services in office, remotely, and in the community, including evenings, while engaging in outreach and education efforts.
Ability To:
- Communicate effectively orally and in writing to interview people, disseminate and record information.
- Understand and follow oral and written directions and apply them in a variety of situations.
- Use correct English grammar, punctuation, and spelling.
Comments:
- Must clear background check for a Community Care Licensed facility, including physical, TB, and drug test.
- Sit for extended periods of time and the ability to manage repetitive hand/wrist movements while using a computer keyboard and/or the phone. Bend, squat, stoop, and walk- up a flight of stairs.
- Lift and/or carry up to 20 lbs., occasionally. Duties are primarily performed in an office environment at a desk or computer terminal.
Work Environment
Typical office and clinical environment. Responsibilities may include offsite meetings, trainings, or other activities as required by business needs.
