Program Description
The Coordinated Specialty Care (CSC) Program provides evidence-based assessment, psychotherapy, medication management, employment and educational support, peer support, and family support services to children and adults who have recently began experiencing symptoms of psychosis as part of a Coordinated Specialty Care (CSC) Team. The CSC Program uses a person-centered, team-based, and recovery-oriented approach to help individuals manage their new experiences while achieving their personal goals
Job Description
Summary/Objective
The Peer Specialist will serve as a peer to the participants primarily receiving CSC services. The Peer Specialist will use their lived experience as someone who has experienced psychosis to advocate for and represent our participants' perspectives while working as part of a Coordinated Specialty Care (CSC) Team. The Peer Specialist will provide individual and group-based peer support services to participants, their support persons, educators, and community members. The Peer Specialist will be thoroughly trained in peer supportive services. This position is community/office/remote based.
Principle Duties and Responsibilities:

- Provides a peer perspective based on lived experience to the provider team, participants, and their caregivers/support persons.
- Attends and participates in weekly team meetings.
- Attends and participates in weekly management meetings.
- Attends monthly consultation meetings.
- Attends and participates in all assigned training for professional, program, and agency development.
- Assists with community outreach and education efforts.
- Maintains a caseload of 40 participants and meets expectations of 66% direct service hours monthly.
- Meets with participants on a monthly, biweekly, or weekly basis depending on participant needs.
- Supports participants in attaining self-directed goals.
- Support participants in connecting to local resources and developing a sense of community.
- Facilitate socialization groups.
- Assist participants to get to and from appointments in the community as needed.
- Rotates in Co-leading Multi-family Groups in the evenings 5:30-7:30pm on a bi-weekly basis.
- Support participants in advocating for themselves in meetings with providers.
- Advocates for the requests of the participants during case discussions in team meetings, case management meetings, IEPs, or meetings with support persons where the service user is typically not present.
- Collaborates and coordinates on a weekly basis with other team staff including but not limited to psychotropic medication provider, mental health therapist, employment and educational specialist, and family peer support specialist.
- Create and maintain a resource binder for distribution to the team and participants.
- Maintain timely weekly progress notes up to insurance standards.
- Submit required clinical documentation within 72 hours of service provided.
- Comply with all federal regulations, Health Insurance Portability and Accountability Act (HIPAA), for the privacy and security of client information.
- Maintain quality assurance standards, adherence to MediCal/MediCare documentation requirements and data collection for measured outcomes.
- Additional duties as needed.
Minimum Requirements
- High School Diploma.
- Lived experience with mental health conditions (psychosis preferred)
- Working knowledge of peer support roles and the history of service user movements.
- Familiarity and knowledge of Sonoma County.
- Willingness to work outside of an office setting, in the community and homes of participants.
- Able to attend Team Meetings Tuesdays 10am-12pm
- Able to rotate in co-facilitating MFG Groups on a bi-weekly basis Wednesdays 5:00pm-7:30pm
- Open to working some afternoon/evenings 2pm – 6pm
- Driver's License.
- Preferred Qualifications:
- Completion of the Medi-Cal Peer Support Specialist Certification (or willingness to obtain).
- Experience supporting people of varying cultural & ethnic backgrounds in navigating the mental health system preferred
- Spanish Speaking preferred

Comments:
- Must clear background check for a Community Care Licensed facility, including physical, TB, and drug test.
- Sit for extended periods of time and the ability to manage repetitive hand/wrist movements while using a computer keyboard and/or the phone. Bend, squat, stoop, and walk- up a flight of stairs.
- Lift and/or carry up to 20 lbs., occasionally. Duties are primarily performed in an office environment at a desk or computer terminal.
Work Environment
Typical office and clinical environment. Responsibilities may include offsite meetings, trainings, or other activities as required by business needs.
