Job summary:


The Spiritual Care Department at Hartford Hospital is known for honoring and providing spiritual care as an essential dimension of healing. Our Chaplains offer spiritual care to patients, families and staff, responding to each person’s unique values and beliefs. Chaplains provide spiritual care to patients, family and staff throughout the hospital. Collaborating with and supporting hospital staff to integrate physical, emotional, spiritual care for the patient and family.

Job Responsibilities:


1.Provides spiritual care to patients, families, and staff at hospital as appropriate and complete spiritual assessments.
2.Collaborates with interprofessional care team, volunteers and faith community to integrate physical, emotional and spiritual healing for the patient.
3.Maintains timely documentation of spiritual care services, which reflect the pertinent information for spiritual assessment, spiritual care involvement, outcomes and recommendations for plan of care.
4.Responds to referrals from staff, patients, and families and well as being responsive to crisis intervention, medical and trauma alerts.
5.Demonstrates strong multi-faith and multicultural chaplaincy skills so as to provide spiritual care in multi-faith and multicultural environment. Respect diversity and differences including, but not limited to, race, culture, gender, sexual orientation and spiritual/religious practices.
6.Provides multi-faith worship experiences at various settings in the hospital.
7.Advocate for the needs and interests of patients including larger system change to improve access to care and improved delivery of services.
8.Maintains ethical principles and professional standards of practice in adherence to APC code of ethics
9.Attends required trainings and department events, including huddles, staff meetings and in-services as required.
10.Participates in ongoing, formal evaluation of practice to assess quality and appropriateness of services, to improve practice, and to ensure competence. Effectively utilizes supervision to improve practice.
11.Demonstrates age specific competence, including knowledge, behaviors and community resources, to meet patients’ needs of all age groups, neonates to geriatric populations; work collaboratively with age-specific hospital-based resources, including Geriatric specialists, pre- and post-natal specialists, and other clinical specialists.
12.Actively participates in Spiritual Care H3W Meetings, generating ideas and works toward solutions.
13.Responsible for continued professional development in accordance with the APC Standards for Continuing Professional Education
14.Provides leadership to ensure access to care, to improve and maintain the quality of care, demonstrated by participation in teams and work groups across HH and HHC. Mentoring others within and outside the spiritual care profession.
15.Participates in the Spiritual Care Council structure, embracing department QAPI process, tracks Spiritual Care dashboard and is aware of departmental metrics. Can describe the HHC balanced scorecard and cascading metrics for Patient Care Services and Spiritual Care Services.


Qualifications



  • MDiv or DMin with focus in pastoral care

  • Graduate theological degree.

  • Must have formal graduate theological education in pastoral care, theology, and/or psychology.

  • One or more years' experience in pastoral ministry and/or hospital chaplaincy

  • Four units of Clinical Pastoral Education accredited by the Association for Clinical Pastoral Education, Inc.

  • Faith group endorsement/ecclesiastical endorsement/ Judicatory Approval

  • Must be able to communicate effectively orally and in writing.

  • Computer literacy.
