We’re seeking a dedicated and compassionate Community Outreach Worker to join our team and help connect vulnerable individuals with essential health and social services.
What You’ll Do:
As a key member of our Outreach Services Team, you will:
Engage directly with adults experiencing homelessness in shelters, on the streets, and other community settings.
Connect clients to critical services like healthcare, mental health care, and substance use treatment.
Provide one-on-one health education and risk reduction interventions, including the safe use of medical supplies and naloxone.
Support clients by accompanying them to appointments and helping them navigate services.
Collaborate with a multidisciplinary team to develop holistic care plans tailored to each individual’s needs.
Assist in mobile clinic operations and street outreach events.
What You Bring:
A high school diploma or GED (Associate’s degree in Human Services preferred)
At least 2 years of experience serving hard-to-reach populations
Strong knowledge of Baltimore City resources and a commitment to harm reduction and trauma-informed care
A personal vehicle, valid Maryland driver’s license, and a willingness to travel throughout the city
The ability to work comfortably in unpredictable, street-based environments
