What You’ll Do
As a key member of our Outreach Services Team, you will:
- Engage directly with adults experiencing homelessness in shelters, on the streets, and other community settings.
- Connect clients to critical services like healthcare, mental health care, and substance use treatment.
- Provide one-on-one health education and risk reduction interventions, including the safe use of medical supplies and naloxone.
- Support clients by accompanying them to appointments and helping them navigate services.
- Collaborate with a multidisciplinary team to develop holistic care plans tailored to each individual’s needs.
- Assist in mobile clinic operations and street outreach events.
- A high school diploma or GED (Associate’s degree in Human Services preferred)
- At least 2 years of experience serving hard-to-reach populations
- Strong knowledge of Baltimore City resources and a commitment to harm reduction and trauma-informed care
- A personal vehicle, valid Maryland driver’s license, and a willingness to travel throughout the city
- The ability to work comfortably in unpredictable, street-based environments
