Responsibilities:
- Work with clients to identify their needs and goals, including conducting assessments and gathering relevant information
- Develop and implement action plans to help clients achieve their goals, including coordinating with other service providers as needed
- Provide ongoing support and monitoring to ensure progress, including conducting follow-up meetings and check-ins
- Maintain accurate and up-to-date records of client interactions and progress
- Collaborate with other team members to ensure continuity of care and services
- Other duties as assigned
- Bachelor's degree in social work, psychology, or a related field
- Minimum of 2 years of experience as a case manager or in a similar role
- Strong communication and problem-solving skills
- Ability to multitask and handle a high-volume workload
- Ability to work collaboratively with other team members
- Valid driver's license and reliable transportation are a plus
