HealthCare Support seeks a Clinical RN / LCSW Case Manager to fill a Fortune Top 25 Managed Care Company opening.
Daily Responsibilities for a Clinical RN / LCSW Case Manager:
- Managing a case load for healthcare members with long-term care needs.
- Monthly and quarterly member contact, including 80% travel. This is a remote role. The position will require a driver’s license.
- Complete assessments with members, caregivers, or providers to obtain information regarding client status, support system, and need for services for care plan development.
- Monitor delivery of services and follow up with members, caregivers, or providers through in-person visits and telephonic contact
- Authorize and coordinate referrals for services.
- Ensure provider services are delivered without gaps and identify functional deficiencies in care plans.
- Assist in coordinating the development of informal or voluntary services to integrate into the member care plan. Collaborate with discharge planners, physicians, and other parties to ensure an appropriate discharge plan and care plan and coordination of acute care and long-term care services!
- Assist members with filing and resolving complaints and appeals.
- Clear and active NC RN or LCSW license
- 2 years of Care Management experience
- Experience with electronic medical health records - please list which EMRs they have used
- Microsoft Office
- Valid driver's license with auto insurance and reliable transportation
- Graduate from an accredited school of nursing or have a master’s degree if LCSW
- Home health
