LPHA/Caseworker II performs clinical services including psychosocial assessments, treatment planning, discharge planning, and individual/group counseling. LPHA/Caseworker II works closely with RN and other treatment team members to develop the most appropriate strategies for individuals at the crisis treatment center. LPHA/Caseworker II may assist with interventions as needed with individuals experiencing challenging behaviors.

EXAMPLES OF WORK PERFORMED:
- Perform crisis and substance abuse counseling per community mental health standards.
- Assist in maintaining a therapeutic environment at the Diversion Center.
- Develop an after-care plan for the individual returning to the community.
- Provided feedback to the treatment team including physicians on behalf of the individual.
- Maintain timely documentation.
- Position requires a Masters Degree in social work, psychology, counseling, marriage and family therapy, or other closely related field.
- License required for LPHA – LPC/ LCSW/ LMFT or Caseworker II – LPC-I/LMSW.
- One year case management experience preferred.
Knowledge, Skills, and Abilities:
- Ability and skill to communicate with staff, individuals, families, and community agencies.
- Must be knowledgeable of community resources for possible referrals.
- Must have knowledge of psychiatric disorders and best practice models of treatment.
- Must have excellent computer skills and knowledge.
- Must have reliable transportation and be able to travel as needed.
- Intervenes with verbal and physical interventions as required within the guidelines of PMAB.
- Must have physical ability to sit for extended periods; working at computers; lifting up to 50 lbs; some stooping, bending and kneeling and drive a vehicle for travel to and from meetings.
- Must have excellent time management skills and excellent clinical documentation skills.
- Must have the ability and knowledge to provide intake assessment and counseling to clients admitted to the Diversion Center.
