SUMMARY DESCRIPTION OF PROGRAM
Heartland Bridges houses two programs, Bridges Transitional Living and Bridges Crisis Care. Transitional Living is a short-term transitional housing program enriched with supportive services designed to assist people with a diagnosed mental health or co-occurring condition while they transition to stability in the community. Crisis Care is a residential stabilization service designed for individuals who need a safe, secure environment for 3-5 days while stabilizing from a crisis after returning to the community where they live.
SUMMARY DESCRIPTION OF WORK
The role of the Case Manager I - Overnight is to provide facility oversight and assist in duties to ensure the day-to-day operations of the program. Decision making, problem solving, and highly developed interpersonal skills are critical.
Compensation : between $16.31 and $19.44 per hour (wage is based on total years of relevant experience)
Work Schedule : Two overnight shifts (20 hours) per week, from 10 : 00 p.m. to 8 : 00 a.m.
MINIMUM QUALIFICATIONS
- High School diploma
- Two years of experience in behavioral or mental health services
- Valid driver’s license and acceptable driving record
