HeartShare St. Vincent's is looking for a Child Advocate RN/LPN
The Child Advocate is responsible for a caseload of children and youth. The Child Advocate makes clinical assessments during visits in the community, provides anticipatory guidance and health-related education to children and families; and helps coordinate care for children living with medical complexity, or disabilities and their families.
The Child Advocate provides these services during regularly scheduled visits to families' homes and other community sites, and on-site in our 29i Medical Practice. Care is delivered in collaboration with HSVS’s team of case planners, behavior specialist, mental health professionals, education specialists, physicians, nurse practitioners, and other staff.
- Responsible for care coordination of medically complex foster children
- Assessing medical records of each child by reviewing child’s diagnosis, birth history, inpatient/outpatient medical records, educational services, and mental health needs.
- Make referrals to community-based care and provide support as needed
- Collaboration with Special Medical Case Planner, Supervisor, and Director. Train and provide medical guidance to foster and birth parents and support for their medical fragile children and family.
- Attend hospital admissions, outpatient clinics, home visits, court and school coordination for these children. Collaborate with health providers, school officials, law guardian, service coordinators- OT, PT, ST, SI. Psychiatrist/psychologist/counselors.
- Assist in quality assurance activities and audits involving services provided to these children. Maintain up to date medical record documentation.
Qualifications
- Registered Nurse Licence required.
- Strong advocacy, communication and organizational skills
- Establish and maintain respectful professional boundaries with individuals and families
- Flexible hours 35-40 hours per week. Individual must be able and willing to work evenings and some weekends.
- Knowledge, understanding, and respect for diverse cultural norms and traditions with clients and co-workers at all times
