- Provides on going discharge planning to facilitate the patient / family caregivers in collaboration with interdisciplinary team to progress toward goals and have a safe timely discharge plan.
- Provides updates to insurances as weekly and as needed to ensure ongoing inpatient approval.
- To be knowledgeable with medicare / Medicaid regulations.
- In consultation with unit professional staff, shall provide supportive counseling to patients and their families, with a focus on helping them cope with traumatic loss.
- Shall elicit from patients and / or their families pertinent information about patients premorbid social, emotional, vocational and spiritual status, and shall communicate this information to the interdisciplinary team in a coherent and relevant manner.
- Shall effectively communicate to the interdisciplinary team any concerns raised by the patient or family
- Shall participate in discharge planning and assist with post-discharge referrals for mental health or substance abuse.
- Utilize knowledge of payer limitations, cost, services and availability of resources to help patients and families make realistic, cost-effective decisions about patient care.
Requirements
Possession of a license and current registration as a Licensed Master Social Worker in New York State.
