General Statement of Duties

Provides screening, counseling, case management, training or other specific treatment or rehabilitation activities as directed for the Jail Diversion Program; does related work as required.
Essential Duties
Serves as agency court, jail and state hospital forensic liaison; responsible for developing release plans for clients with mental illness at intercept 4; assisting with securing housing for released inmates, responsible for providing case management to court identified cases; collaborating with legal system to include jails, court, probation and parole; conduct collateral contacts with family members, agency staff and other service providers to include half-way houses, substance abuse treatment facilities, nursing homes and assisted living facilities; provide case management services based on best practices; interventions to include monitoring symptoms, assisting clients with obtaining medication, resource location, linking, coordinating, accessing, planning, education, and providing supportive counseling; knowledge of understanding of principles of rehabilitation and the justice system; court, and jail services to help reduce length of incarceration and increase access to treatment; manages case load independently and works in collaboration with Mental Health Jail team, Mental Health Diversion team, Sheriff's office, court system, state hospital system and community providers.
Knowledge of principles, practices and methods of treatment and / or rehabilitation of individuals with an intellectual disability or mental illness; knowledge of mental health and / or intellectual disabilities and related problems; good interpersonal skills which include the ability to empathize, demonstrate respect, communicate, reflect feelings and confront constructively; ability to relate freely to and work effectively with clients and professionals from all levels of the community; adherence to basic ethics appropriate for any helping relationship; ability to write concisely and coherently; demonstrated ability to use good judgment.
Minimum Qualifications
Possession of a bachelor's degree in a behavioral science; OR, any equivalent combination of experience and training which provides the required knowledge, skills, and abilities.
Additional Information
Valid driver's license issued by the Commonwealth of Virginia is required. QMHP certification required; TB Screening and Child Abuse Registry Check required upon hire.
