General Statement of Duties
Closing Date:

Provides screening, counseling, case management, training or other specific treatment or rehabilitation activities as directed for the Youth and Family Services team; does related work as required.
Essential Duties
Provide case management services to youth. Coordinate, link, assess and monitor client services. Provide Intensive Care Coordination services to families to decrease their need for hospitalization. Utilize the Wrap Around Model for providing Intensive Care Coordination. Provide crisis response to clients and linkage to appropriate community resources. Act as liaison for the family with various community stakeholders, including courts, schools, CSA, and other local agencies. Advocate for clients and families. Communicate with medical professionals to include psychiatrists to monitor client response to medication. Facilitate parenting groups in the evenings. Meet with clients in their homes, the school, or the office as is convenient for the family. Maintain all clinical, case management, and administrative documentation as required.
Knowledge of principles, practices and methods of treatment and/or rehabilitation of individuals with an intellectual disability or mental illness; some knowledge of mental health and/or intellectual disabilities and related problems; good interpersonal skills which include the ability to empathize, demonstrate respect, communicate, reflect feelings and confront constructively; ability to relate freely to and work effectively with clients and professionals from all levels of the community; adherence to basic ethics appropriate for any helping relationship; ability to write concisely and coherently; demonstrated ability to use good judgment.
Minimum Qualifications
Possession of a bachelor's degree in a behavioral science; OR, any equivalent combination of experience and training which provides the required knowledge, skills and abilities.

Possession of valid driver's license issued by the Commonwealth of Virginia is required. TB Screening and Child Abuse Registry Check required, upon hire.
