GENERAL SUMMARY:
The Associate of Case Management's responsibility is to provide patient care management services for identified behavioral health patients who are currently involved in a Henry Ford Allegiance Health Behavioral Health program. The Associate of Case Management will engage identified behavioral health patients in a collaborative process of identifying, planning, accessing and advocating for, coordinating, monitoring and evaluating resources, supports and services necessary to optimize their functioning and well-being. This includes coordinating services within and between HFAH Behavioral Health programs, providing and coordinating referrals to needed community services and resources and facilitating transitions of care to maximize effectiveness of care and a positive patient experience. The Associate of Case Management will function as a core member of a collaborative care team that involves the patient's primary treatment provider, a consulting psychiatrist and other health providers that may be involved in the patient's inpatient and outpatient behavioral health care.
EDUCATION/EXPERIENCE REQUIRED:
Bachelor's degree required. Bachelor of Social Worker license in the State of Michigan preferred. Experience working with patients who have co-occurring mental health, substance use disorder, and physical health concerns. Discharge planning and behavioral health case management required. Exceptional computer and analytical skills. Ability to establish priorities and coordinate work activities among multiple requests. Must be capable of standing, walking, stooping, bending and lifting up to 25 pounds. Minimum of two years of experience working with behavioral health patients in an ambulatory and/or community health setting preferred. Working knowledge/experience in utilization management, managed care, and payer issues preferred.
CERTIFICATIONS/LICENSURES REQUIRED:
State of Michigan Social Worker license if applicable.
