The Case Manager is responsible for coordinating and providing individualized support, resources, and services to clients in need. They assess client needs, develop care plans, and collaborate with various service providers to ensure effective case management and positive outcomes.
Key Responsibilities
- Assess clients’ needs, situations, strengths, and support networks to determine appropriate resources and interventions.
- Develop and implement individualized care plans to address clients' goals and challenges.
- Coordinate services and resources, including healthcare, social services, housing, financial assistance, and employment support.
- Monitor client progress through regular follow-ups and adjust care plans as needed.
- Advocate for clients’ rights and needs within the system to ensure they receive appropriate support.
- Maintain accurate and confidential case records, documentation, and reports.
- Collaborate with multidisciplinary teams, including social workers, healthcare providers, legal professionals, and community organizations.
- Educate clients and their families on available resources, rights, and responsibilities.
- Ensure compliance with legal, ethical, and regulatory standards in case management practices.
- Conduct crisis intervention and provide emotional support to clients in distress.
Hours & Location
- Monday- Friday: 8am- 4:30pm
- Onsite, with future WFH opportunities
Qualifications and Skills
- Bachelor’s degree in Social Work, Psychology, Human Services, or a related
- Relevant certification (e.g., Certified Case Manager (CCM), Licensed Social Worker (LSW), or other state- specific credentials) is a plus.
- Previous experience in case management, social work, or a related field.
- Strong interpersonal, communication, and problem-solving skills.
- Ability to work independently while collaborating with multidisciplinary teams.
- Knowledge of community resources, social services, and healthcare systems.
- Proficiency in maintaining confidential client records and documentation.
- Compassionate, patient, and dedicated to client advocacy.
