Job Summary
Performs individualized client/patient care activities and related household services involved in caring for the personal needs and comfort of clients/patients.
Principal Duties & Responsibilities
- Assists patients with activities of daily living including bathing, grooming, dressing, oral care, etc.
- Follows Plan of Care (POC). Duties include changing, assisting with toileting needs, ostomy and catheter care, applying ted hose/ace wraps, assisting with exercises, meal preparation, and assisting with medications that are self-administered.
- Observes and maintains accurate record of patient’s physical and mental condition; reports any unusual signs or changes in patient’s condition or behavior, accidents, and/or injuries to appropriate nursing personnel.
- Builds and maintains trusting relationships with patients and their families/caregivers; provides emotional support as needed.
- Instructs patients and their families/caregivers regarding patient care.
- Maintains communication with other disciplines and family members/caregivers to facilitate quality patient care.
- Performs light housekeeping of patient’s home such as making beds, changing linens, removing trash, washing dishes, etc.
- Completed documentation will be submitted within required timeframe.
- Participates in Holland Home and Faith Hospice in-service programs to maintain and improve specialized skills.
- Attends staff meetings and discipline specific meetings as required.
- Performs patient care in a compassionate manner in accordance with the mission and values of Holland Home.
- Understands patient rights and incorporates them into daily practices.
- Supports and models by conduct the Holland Home Code of Conduct to patients, employees and public.
- Meets Holland Home’s standards for confidentiality.
- Utilizes the Positive Approach to Care™ methodology in accordance with the mission and values of Holland Home.
- Assures patient has necessary medical supplies.
- Identifies safety hazards and emergency situations and initiates corrective action as appropriate.\
- Is responsible to obtain information presented at staff meetings and discipline specific meetings when unable to attend.
- Attendance at required annual in-services is mandatory.
- Weekend and/or Holiday hours may be required.
- Performs other duties as assigned.
- The job requires knowledge normally acquired through a general high school diploma or equivalent plus completion of a state-approved nursing aide training program.
- Experience working with the elderly is desirable.
- Experience working with hospice patients is desirable.
- Current Michigan CNA certification is required.
- Reliable transportation is required.
- Proof of a valid Michigan driver’s license and vehicle liability insurance is required. The incumbent is also required to have reliable transportation to make home visits
- Ability to work with patients who are at end of life.
- Interpersonal skills necessary to communicate effectively with members of the interdisciplinary team, patients and their families in situations requiring courtesy, tact, respect and patience.
- Written and computer skills necessary to maintain required documentation.
- Ability to read, speak, write and understand the English language.
- Ability to meet Holland Home’s standards for attendance.
- Ability to meet the general health requirements as set forth by the polices of Holland Home, which includes a physical examination, TB testing and seasonal flu shot.
- Pressures related to completing required documentation in a timely manner and dealing with the concerns and emotional needs of clients/patients and their families/care givers.
- Ability to transfer/ lift/ turn/position/assist clients/patients, reach/stoop/bend/kneel/crouch, and utilize the equipment and devices necessary in providing prescribed care to clients.
- Ability to access homes of clients/patients which may not be barrier free.
- Normal client/patient care environment with minimal exposure to infectious conditions and communicable diseases when using standard precaution methods.
- Work involves travel to homes of clients/patients with occasional exposure to adverse weather conditions.
- Occasional exposure to potential undesirable environments.
