What Will You Do?
In This Community-based Program, Our Current RN Case Managers
- Work a Monday – Friday daytime schedule
- Participate in a back-up on call rotation with the other RNs on the team; this equates to the equivalent of one calendar week every 4 months (5pm – 10pm on weeknights and 9am – 9pm on Saturday and Sunday)
- Manage a case load of hospice patients within an assigned geographic territory to minimize drive time and spend more time with your patients (we cover a 35-mile radius around Grand Rapids)
- Flexibility, resiliency, and adaptability
- Self-directed and autonomous
- Ability to prioritize
- Organized
- Comfort with navigating various technologies and computer applications
- Team player mentality
- RN licensure in Michigan
- At least 2 years of med/surg or acute care nursing experience
- Hospice and/or hospital experience preferred
- Passion for hospice nursing
