The Health Outreach Manager reports to the CEO and collaborates with the other program directors and teams to coordinate services to address and meet the needs of the clients addressing health access, navigation, and evaluation of all social determinants of health to optimize and promote client wellbeing. The scope of the work is to provide bilingual, bicultural CHWs – community health workers (Promotor[a]) services to the underserved community. This position supervises the CHWs in the program as well as BSW students. Coordinates with other Holy Cross Ministries’ departments and other community organizations to assist in meeting the biopsychosocial and health needs of those served in an effective and efficient manner.
Duties:
- Acts a promotor(a)/CHW and is able to carry out the principal accountabilities of a Holy Cross Ministries Promotora/CHW as needed.
- Uses data dashboard for decision-making and program fidelity and improvement and uses on-going performance evaluation(s) for the outreach services program.
- Participates and/or conducts regular meetings with staff for purposes of program and administrative updates, program in-service, communication between staff, review of casework and professional development and education.
- Follows HCM personnel procedures regarding employment practices specifically related to employment. Adheres to all Federal and State laws and regulations.
- Serves as a member of HCM staff committed to fulfilling the mission of HCM by serving program participants, clients and each other in an atmosphere of hospitality, which is characterized by cooperation and a positive, respectful relationship with each individual.
- Performs other duties in a timely and appropriate manner as may be assigned by supervisor.
- Supervises and coordinates promotoras as they carry out their principal accountabilities including oversight of data quality.
- Participates in the identification of service needs through assessments, case finding and problem solving. Coordinates with social, economic and health agencies and organizations.
- Coordinates these findings with the other program directors, the management committee, and the promotoras to develop and evaluate the effectiveness of the program. Is capable of working effectively with agencies where Promotoras report daily.
- Assists in the development and/or implementation of an outreach & Community Health Worker curriculum and provides for leadership training and development for the promotoras in order to better meet the needs of the populations served. Has the ability to articulate program and staff needs.
- Will effectively interface with HCM Communications and Development to provide necessary information for fundraising, grant writing, and marketing purposes to fund and promote the program.
- Coordinating with other program leads advocating for the health and wellbeing of underserved communities. Build trust and rapport with community members through advocacy.
- Assists in the development and implementation of program evaluation and quality improvement through use of performance and impact measures.
Qualifications:
- Bachelor’s degree in social work or human service-related field or equivalency is required.
- 3-5 years’ experience as a CHW or in a social service-related field as well as completed the Utah CHW training modules.
- Bilingual in Spanish is preferred.
- Supervisory and administrative responsibilities as well as experience working with internal and external groups and in program development and planning is preferred.
- Excellent communication, interpersonal, and organizational skills.
- Strong reporting capabilities for tracking efficiency and deliverables.
- Strong advocacy skills and a commitment to addressing health disparities.
