RN Case Manager with **$10,000 Retention Bonus**

Under the direction of the Patient Care Manager, the RN Case Manager for Home Healthcare, Hospice & Community Services (HCS) provides and directs the provision of home nursing care, based on agency policy and procedures, through the competent use application of the nursing process.
This is a full-time, benefited opportunity that works, Monday - Friday, 37.5 hours per week, serving the Peterborough, NH area.
Responsibilities include, but are not limited to:
- Assumes responsibility and accountability for the practice of professional nursing in a manner consistent with the NH State Nurse Practice Act and American Nurses Association Standards for Home Health Nursing.
- Implements teaching appropriate to the client’s needs and within the context of his or her environment and value system
- Practices nursing with respect for individual, cultural and spiritual differences
- Coordinates delivery of care along health care continuum utilizing community resources
- Demonstrates teamwork and effective communication to accomplish client, team and agency goals
- Graduate of a state-licensed basic nursing program
- Preparation in Community Staff Nursing through a National League for Nursing approved program
- With at least one (1) year professional nursing experience under appropriate nursing supervision; or
- A combination of education and experience that demonstrates the person has acquired the necessary competencies required for the position
- Possession of a current State of New Hampshire license to practice as a licensed practical nurse
- Maintain current cardiopulmonary resuscitation (CPR) certification
- Must be able and willing to learn to utilize a portable electronic field device. Must be able to manage electronic patient health information (ePHI) remote access in accordance with HCS' policies and procedures
- Sensitivity in human relationships and able to cope with family emotional stress and tolerant of individual lifestyles
- Skill in using psychological principles in both one on one and group situations
- Knowledge of measures to comfort the dying. Emotional stability, flexibility and respect for various religious and spiritual beliefs
- Valid driver’s license and access to an automobile during working hours that meet the State of New Hampshire's registration regulations and auto liability insurance.
