Essential Job Duties and Responsibilities:
- Making periodic home visits to clients to:
- Perform start of service assessments, develop the client’s initial personal care plan of care, obtain necessary consents, and conduct re-assessments as required by COMAR and agency policy;
- Determine the type of caregiver required (if applicable), assess caregiver’s skills and evaluate ability to understand and carry out the client specific plan for personal care;
- Discuss the proposed plan of care with the client/family and the assigned caregiver;
- Identify the conditions and incidents which the caregiver should bring to the agency’s attention;
- Monitor personal care service performance as required by COMAR and agency policy and oversee delegated nursing duties (including medication administration) provided by the caregiver (if indicated in the Plan of Care/Plan of Service);
- Observe interactions and the relationship between the client and caregiver and consult with the client, family members, client’s physician (in case of necessity), case managers, support planners, and other parties;
- Assess client’s medical condition, document condition changes, communicate findings to the agency, and provide recommendations for other services as needed;
- If applicable, adheres to client specific plan of care in conformance with physician’s orders.
- RNs shall also be required to:
- Devise accurate and concise reports and assures timely submission of reports and forms in a manner prescribed by the agency;
- Ensure overall compliance with agency policies and procedures, and program specific rules and regulations;
- Provide dependable, reliable and compassionate service to clients;
- Be accessible and responsive to Clients, Supports Planners, and PHC staff,
- Keep source documentation on Services provided as required by PHC records policies.
- Upon instruction from agency and if qualified, RN may be involved in provision of additional services, such as family, caregiver, and consumer training, behavioral consultations, and CPR/First Aid training.
- Qualifications:
- Accredited Associate or Diploma nursing school graduate, bachelor’s degree preferred;
- Current and unencumbered license to practice as a Registered Nurse in the State of Maryland;
- Experience as a registered nurse; home care experience strongly prefered;
- Bilingual candidates are encouraged to apply.
- Ability to work independently;
- Maryland driver’s license and a minimum of one (1) year clean driving history;
- Reliable transportation;
- Ability to successfully complete required background check;
- Excellent writing skills, experience with electronic medical record preferred;
- A positive attitude with a commitment to the clients we serve.
Pay: $65 per visit for revisit/$75 per visit for first visit.
