Summary of Position: This is a contract position with the opportunity to go full time W2. The Qualified Mental Health Professional (QMHP) is responsible for providing Targeted Case Management and Mental Health Rehabilitative Services to individuals and their families.

• Services may be provided in the participant's home or appropriate community setting.
• The QMHP will deliver case management and skills-based interventions targeted increasing the individual's natural supports and strengths, fostering resilience and recovery.
• The QMHP is responsible for accurately documenting services, attending Wraparound Team meetings as needed, and communicating participant progress to supervisors, team members, and the referring authority.
• Primarily teaching skills and providing case management services.

Bi-Lingual (Spanish Speaking) is a plus but not required.

Qualifications: Bachelor's or Master's degree

Responsibilities:

The Contractor will be required to perform the following services while under contract:

- Client Targeted Case Management Services
- Client Treatment Plans (Tx)
- Client Progress Notes/ Client Comments
- CANS/ ANSA Assessment
- Attend Clinical Supervision and Staff Meetings

QMHP Certification

• Knowledge of evidence-based treatment approaches for various mental health conditions.
• Strong assessment and diagnostic skills.
• Excellent verbal and written communication abilities.
• Demonstrated ability to work effectively both independently and as part of a team.
• Empathy, compassion, and a non-judgmental attitude towards clients.
• Proficiency in electronic health records (EHR) systems.
• Experience in the provision of home and community-based services.

Job Type: Contract

Pay: $25.00 - $27.00 per hour
