Compensation: up to $22.505 per hour

Job Status: Non-Exempt / Hourly
Compensation: up to $22.505 per hour
Responsibilities include:
- Screening and assessing referrals to our Supportive Housing programs
- Presented completed intakes to CARES Committee
- Verifying HUD eligibility
- Admitting individuals and families into Permanent Supportive Housing program
- Preparing annual review for Permanent Supportive Housing grants and all other Federal grant documentation requirements
- Completing a financial review with residents upon admission
- Providing case management services to support residents' links to community services
- Conducting home visits as needed
- Overseeing the program to ensure compliance
- Other duties as assigned
- One-year experience in working with people with Substance Use Disorders, homelessness and case management services preferred. Excellent communication skills - both written and oral - are required. Counseling experience is a plus
- Valid Driver's License and Reliable Transportation required
- Associate's Degree required
- CASAC/CASAC-T or other QHP preferred
- 11 paid holidays per calendar year
