SUMMARY: The Occupational Therapist (OT) will implement the "CAPABLE Program" after completion of required training through Johns Hopkins University School of Nursing CAPABLE training. The OT will assess and implement strategies at people's homes who have agreed to participate in CAPABLE. The CAPABLE program consists of up to six occupational therapy contacts with program participants. The OT meets with participants in the person’s home unless otherwise determined. The OT uses the training and Motivational Interviewing techniques to conduct a semi-structured clinical interview to identify and prioritize problem areas. The OT captures the goals of the participant in up to 3 Action Plans and creates a Work Order for the Handy Worker. The OT interacts and communicates with the CAPABLE RN and other team members as needed.

Qualifications
QUALIFICATIONS: Bachelor's degree in Occupational Therapy. Licensure from Board of Occupational Therapy required. Ability to work from a collaborative, client-centered model. Willingness and ability to implement a protocol and balance with clinical judgment. At least one year experience as a licensed Occupational Therapist required. Preferred Qualifications include experience with home care with older adults highly preferred.
PHYSICAL DEMANDS: The physical demands of the Occupational Therapist requires moderate visual concentration for patient charting/documentation. This position carries a risk of exposure to infectious diseases and requires skill in standard safety precautions. The Occupational Therapist requires the mobility to ambulate or lift a patient with or without the assistance of devices in excess of 75 pounds using proper body mechanics.
WORK ENVIRONMENT: This position works 30-40 hours per week, with occasional requests to work additional hours as needed. At times, due to patient census and acuity, this position may be required to remain on site at Hospice House during meal breaks. The Occupational Therapist position may be required to provide care in what has been determined by local authorities as a high-risk crime area and unsafe situations (potential or actual). The Occupational Therapist must be able to accurately, timely and concisely document the patient/family encounter using a computer. The Occupational Therapist is required to perform patient visits utilizing their personal vehicle.
