Program Description:
In conjunction with the Lawrence County MH/DS office and existing county forensic initiatives, Human Services Center will work to identify individuals with a co-occurring forensic status and mental health concerns in need of stable housing and mental health care coordination. The Adult Forensic Coordinator will work in conjunction with existing Lawrence County forensic initiatives to ensure care coordination, medical assistance benefit coordination, resource coordination, and housing coordination for individuals as a part of release from incarceration, or diversion from incarceration.
This program is designed to reduce forensic involvement and recidivism, while helping the individual establish community supports, vocational services, and stable housing to improve their quality of life.
Staffing Pattern:
The Adult Forensic Coordinator position will be a full time position with non-traditional hours and days. The Adult Forensic Coordinator will possess a minimum education level of a bachelor’s degree in social sciences, experience with service coordination. Experience with the forensic population is preferred.

Responsibilities include:
- Works collaboratively with the Lawrence County MH/DS and their affiliates by providing Adult Forensic Coordination services for individuals with mental health concerns being released from incarceration, or as part of a jail diversion plan
- Provides daily oversight for the residents in the Human Service Centers Forensic Bridge Housing Program
- Coordinate a Diagnostic Assessment if not actively receiving outpatient services, or a follow-up appointment for someone whom is established with a provider
- Coordinates benefits for individuals through the Department of Human Services, Medical Assistance office
- Interview possible participants at various locations, including the Lawrence County Jail
- Develops service plans in consultation with consumers, families and juvenile probation, and ensures implementation of all provisions and goals of the treatment plan as developed prior to the consumer’s discharge from residential placement.
- Assesses consumer’s needs based on strengths and needs and assists individuals in maintaining or improving functioning in the following areas: reducing the number of police contacts with the individual; reducing recidivism rates; restitution/accountability; educational/vocational skills; social supports; family supports; independent living skills.
- Maintains ongoing communication, cooperation and collaboration with the entities charged with oversight related to the forensic involvement of the individual, i.e. adult probation, judicial system, etc.
- Participates as a member of a multi-disciplinary assessment team to find ways to best assist the individual
- Provides support and psycho-educational services to consumers
- Be part of an on-call rotation with other supervisors to provide 24 hour emergency coverage to the Forensic Bridge Housing program
- Maintains a case load as determined by Human Services Center supervisor and based on admission into Forensic Bridge Housing
- Assists individuals in identifying and utilizing resources including: natural supports (family, peers, neighbors); educational/vocational rehabilitation; community services; specialized services such as mental health treatment
- Supplies documentation, as permitted, regarding consumer’s progress toward treatment goals to court officials, Parole and/or Probation staff
- Attends court hearings and provides verbal testimony about consumer’s progress in the program
- Uses a lap top computer for job functions related to documentation of service delivery and reports.
- Upon receipt of referral, communicate the individuals care needs to the Director of Assessment and Outpatient Services
- Will complete additional responsibilities as directed by the Executive Director and/or their designee

Pay: $45,000.00 - $49,000.00 per year
