Become a part of our caring community and help us put health first The Associate Director of Service Coordination leads Managers and teams of Service Coordinators (nurses, behavior health professionals, direct service professionals) responsible for members integrated care planning. The Associate Director requires a solid understanding of how organization capabilities interrelate across department(s). The Associate Director, Care Coaching · oversees the evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and well-being of members. · Decisions are typically related to identifying and resolving complex technical and operational problems within department(s) · could lead multiple managers or highly specialized professional associates Use your skills to make an impact Required Qualifications: Candidate must possess one of the following: -Bachelor’s level degree in nursing with an active RN license in the state of Indiana -Masters level, licensed social worker or -Masters level degree in health and human services 2 years of experience working with the elderly or disabled population 2 years of previous management/supervisor level experience Proficiency in analyzing and interpreting data trends Comprehensive knowledge of all Microsoft Office applications, including Word, Excel and PowerPoint Preferred Qualifications: Experience with long term services and supports (LTSS) and home and community-based service coordination as it relates to implementing practices to improve social determinants of health Knowledge about Indiana community resources, and prior experience with the program population and informal caregivers WAH Internet Statement To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested Satellite, cellular and microwave connection can be used only if approved by leadership Humana will provide Home or Hybrid Home/Office employees with telephone equipment appropriate to meet the business requirements for their position/job. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $84,600 - $116,300 per year This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
