Job Description
Job Title : LPN - Full Time Day / Evening Rotation
Job Specialty : Long-Term Care

Shift : Day / Evenings rotation (5x8 per week, with potential for 3x12s based on availability), Every Other Weekend and Holidays required

Experience : 1 year in an LPN capacity at a Long-Term Care Facility
License : NY LPN Licensure
Certifications : BLS (certified through American Heart Association or RQI)
Must-Have : Demonstrate knowledge and skills necessary to provide care appropriate to adult and geriatric patient groups
Job Description :
- Provide both direct and indirect skilled nursing care for assigned residents.
- Follow defined policies and procedures under the supervision of a Registered Nurse.
- Maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff.
- Engage in continuing education and in-service programs to uphold an appropriate level of knowledge and clinical expertise.
- Ability to assess and collect data regarding residents' physical and psychosocial conditions on an ongoing basis, and communicate findings to the RN and other members of the Interdisciplinary Team.
- Contribute to the development of a culturally relevant and age-appropriate plan of resident care that mirrors nursing diagnosis and interventions for optimal resident outcomes, involving the resident / family and healthcare team in this process to promote a healthy, safe environment.
- Recommend revisions to the care plan based on evaluation of resident outcomes and indicate the need for referrals to address all resident care aspects.
- Collaborate with the resident / family and Interdisciplinary Team members within the nursing practice scope to ensure customer satisfaction.
- Maintain current knowledge and competency in nursing practice.
- Participate in the mentoring / orientation of staff, and consider factors such as safety, cost, and impact when planning and delivering care.
- Provide leadership in the professional practice setting under the RN's guidance and demonstrate knowledge and competency.
- Consideration of occupational exposure to blood-borne pathogens as tasks involve potential contact with blood, body fluids, or tissues.
