Job Title : LPN Full Time D / E Rotation

Experience : 1 year experience as LPN in a long-term care facility
License : NY LPN Licensure
Certifications : BLS (certification through AHA or RQI specifically)
Must-Have : Experience in a long-term care facility, ability to work every other weekend and holidays
Job Description :
- The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit.
- Must demonstrate knowledge of the principles of growth and development of the life span and possess the ability to assess data reflective of the patient's status.
- Interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs and provide the necessary care to adult and geriatric patients.
- Provides direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse, following defined policies and procedures.
- Maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff.
- Maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs.
- Ability to provide care to geriatric residents.
- Collect data relevant to the resident's physical and psychosocial condition on an ongoing basis and communicate findings to RN and other members of the Interdisciplinary Team.
- Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions to achieve desired resident outcomes.
- Involve the resident / family and healthcare team in the care process while promoting a healthy, safe environment.
- Recommend revisions to the plan of care after evaluating resident outcomes and indicate the need for referrals to ensure all aspects of the resident's care have been addressed.
- Collaborate with the resident / family and members of the Interdisciplinary Team within the scope of nursing practice, assuring customer satisfaction.
- Maintain current knowledge and competency in nursing practice and participate in the mentoring / orientation of staff.
- Consider factors related to safety, cost, and impact when planning the delivery of care.
- Under the direction of the RN, provide leadership in the professional practice setting.
- Demonstrate knowledge and competency, with occupational exposure to blood-borne pathogens reasonably anticipated.
- Required tasks involve potential contact with blood, body fluids, or tissues.
