What are the responsibilities and job description for the Care Management Outreach Supervisor position at ICAN Inc?
The Care Management Outreach Supervisor is an integral part of the Adult and Children’s Care Management team. Some of the core functions of this position are to locate, educate, engage, and consent new patients referred to the both Adult and Children’s Health Home. The Outreach Supervisor adheres to and promotes the philosophy and missions of the company by performing the following duties and responsibilities.
Duties & Responsibilities:
Conducts electronic, phone, mail and in-person outreach to search for, locate and engage consumers.
Supervises Outreach Specialists in both the Adult and Children’s Health Home Programs. Provide Supervision and oversight of cases, paperwork, eligibility and assessments.
Ensures that the Outreach Team conducts home visits and calls to engage prospective clients in a discussion around Health Home program benefits and enrollment.
Works with Outreach Specialists to schedule appointments to enroll prospective clients into the Health Home Program.
Along with the Care Management Outreach Manager will be Point of Contact for referrals coming into The Adult and Children’s Health Home Programs.
In collaboration with Outreach Specialists, helps members identify and participate in community-based activities/resources that support their physical or behavioral health as part of their individual care plan.
Assists clients with scheduling appointments as requested by the Outreach Team, Adult Care Coordinators and Youth Care Coordinators.
Completes detailed written records of contact with clients in a timely manner utilizing electronic record systems.
Ensures compliance of both Outreach and Enrolled members in the Health Home Program. Report any issues affecting compliance, engagement and enrollment.
Ensures the Outreach Team members are completing Enrollment documentation and Assessments accurately and timely.
Achieves an engagement rate comparable or above industry standard.
Additional duties may be assigned, on an as needed basis.
Education/Experience:
Bachelor’s Degree in Human Services, Social Services, Psychology or related field preferred.
At least 2 years relevant experience plus a minimum of 1-year experience as an Adult or Youth Care Coordinator/ Care Manager.
A valid NYS Driver's License is required.
