Job Title: Medical Assistant / Community Health Worker
Bilingual: English/Spanish
Reports To: Chief Clinical Officer
Summary: Supports Day-to-Day Operations of both Clinical and Administrative duties
Overview: We are seeking a skilled Medical Assistant to join our healthcare team. The ideal candidate will assist healthcare providers across multiple clinics, ensuring efficient patient care delivery.
Duties and Responsibilities:
Supports Day-to-Day Operations/Administration
· Answers and screens office phone calls
· Greets visitors and guides them to appropriate area or person
· Runs monthly clinic reports
· Assures smooth operations of assigned clinics
o Handle incoming inquiries and schedule appointments.
o Create patient files in Practice Fusion
o Manage incoming support documentation.
o Check patients in with vital signs and chart completion.
o Assists in procedures as needed
o Check out patients, reschedule
o Organizes and maintains cleanliness of office and exam rooms
· Scans documents into our EMR system
· Records patient payments into EMR & SwipeSimple
o Reconcile payments at end of day
· Maintains a safe, secure, and healthy environment
· Keeps supplies ready by inventorying stock, placing orders, and verifying receipt
· Keeps equipment functional by following operating instructions and performing preventive maintenance
· Updates job knowledge by participating in educational opportunities
- Serve as a health navigator, working one-on-one with patients who screen positive using a Social Determinant of Health (SDoH) screening tool
- Assist patients in health insurance (Medicaid or ACA) and/or prescription assistance enrollment
- Assist patients in connecting with community resources Actively participate in required training and professional development activities
- Actively participate in key meetings; internal (weekly-local and monthly-group) and external/community (Unite Us, IHN and other organizations, as appropriate)
- Develop and maintain collaborative referral relationships with key community agencies. Include progress in monthly report
- Identifies strengths and barriers of current referral and management systems. Assertively works to build on strengths and resolve barriers to promote positive health outcomes
- Patients screening positive/requesting assistance, will have their needs addressed at the time of their appointment or they will be contacted within two working days via phone by a CHW
- Other duties as assigned
Qualifications: To perform this job successfully requires flexibility, and the ability to communicate well with all levels of staff (internal and external). Must be able to work independently to meet deadlines, and work with team members on projects. Must demonstrate excellent oral and written communication. Computer knowledge is essential. Must be able to maintain confidential information and assure discreet handling of all business and patient related information. Experience working in a non-profit setting is helpful.
Education/Experience: One to two years related experience and/or training; or equivalent combination of education and experience. Two-year degree is preferred. Experience in a medical setting is preferable.
Language and Reasoning Ability: Ability to communicate effectively with all levels of organization and patients is a must. Has the ability to read business correspondence, and procedure manuals. Must be able to effectively present information and respond to questions from groups of managers, clients, customers, and the public.
Bilingual preferred: English/Spanish
Math Ability: Knowledge of basic math skills, and ability to work with spreadsheets (Excel and Access).
Computer Skills: Word processing skills (Microsoft Office products – Word, Excel, Access) are essential, as well as internet research abilities. Knowledge of Publisher preferable. Must be willing to update computer skills as required. Willingness to learn through on-line coursework offered through administrative staff.
